salvia miltiorrhiza pretreatment on the CCK and VIP expression in hepatic ischemia-reperfusion-induced digestivetract congestion

摘要: The inhibitory effect of different reperfusion periods 45 min following hepatic ischemia on the expression of cholecystokinin (CCK) and vasoactive intestinal peptide (VIP) in the jejunum and the effect of salvia miltiorrhiza pretreatment were investigated, and the possible mechanism and implications were explored. Eighty rats were randomly divided into four groups: normal control group (CO group), sham-operated group (SO group), ischemia/reperfusion (I/R) injury group (IR group) and salvia miltiorrhiza pretreatment group (SM group). The rat model of I/R was established by using a non-invasive artery clamp to clip (45 min) or relax the hepatic pedicle. In the SM group, saline (40 mL/kg) and salvia miltiorrhiza injection (6 g/kg) were injected via the tail vein 30 min before clipping the hepatic pedicle. In the SO group only the porta hepatis was dissected after laparotomy without clamping the hepatic pedicle. At 0, 3, 12, 24 and 72 h post-reperfusion, respectively, upper jejunum samples were taken for immunohistochemistry of CCK and VIP. It was found that 0 h after I/R, the expression of CCK and VIP in the upper jejunum was upregulated. With prolongation of the reperfusion period, the expression of CCK and VIP was also increased, reached the peak at the 24th h, and gradually returned to the normal level at the 72nd h after reperfusion. The levels of both CCK and VIP in the SM group were lower than those in the IR group. It is suggested that the digestive tract congestion injury caused by liver ischemia can upregulate the expression of CCK and VIP in the jejunum following reperfusion. Salviae pretreatment can partly reduce the increased expression of CCK and VIP in the jejunum in the same period, which might contribute to the early recovery of gastrointestinal motility.

An approach to locational marginal price based zonal congestion management in deregulated electricity

摘要: Congestion of transmission line is a vital issue and its management pose a technical challenge in power system deregulation. Congestion occurs in deregulated electricity market when transmission capacity is not sufficient to simultaneously accommodate all constraints of power transmission through a line. Therefore, to manage congestion, a locational marginal price (LMP) based zonal congestion management approach in a deregulated electricity market has been proposed in this paper. As LMP is an economic indicator and its difference between two buses across a transmission line provides the measure of the degree of congestion, therefore, it is efficiently and reliably used in deregulated electricity market for congestion management. This paper utilizes the difference of LMP across a transmission line to categorize various congestion zones in the system. After the identification of congestion zones, distributed generation is optimally placed in most congestion sensitive zones using LMP difference in order to manage congestion. The performance of the proposed methodology has been tested on the IEEE 14-bus system and IEEE 57-bus system.

Detection of digestive malignancies and post-gastrectomy complications via gastrointestinal fluid examination

摘要:

To date, gastric carcinoma (GC) is one of the common and fatal digestive malignancies worldwide. The prognosis of GC is not always satisfactory because of late diagnosis. Scholars are keen on discovering novel accurate and economical biomarkers in body liquids for GC screening to detect and evaluate the lesion before the results of imaging techniques are obtained. While traditional serum assays have limited sensitivity and specificity, gastrointestinal juice may provide relevant specific biomarkers because of its close contact with the tumor. Herein, the current progress in the relationship between gastrointestinal fluid analyses and GC is systematically and comprehensively reviewed. The detection of gastric juice pH, fluorescence spectrum, cytology, Helicobacter pylori-associated markers, nitrosamines, conventional tumor markers, amino acids, proteomics, microRNAs, long noncoding RNAs, protein-coding genes, vitamin C, etc., and combination tests of different category markers could provide important diagnostic and prognostic clues for gastrointestinal diseases. Particularly, early GC may be efficiently screened using gastric juice. Gastrointestinal fluid examination could also predict the adverse effects of postgastrectomy, such as pancreatic leakage, fistula, and abscess. Gastric fluid markers should be further studied to reveal the early predicators of malignancy and complications. The methods for obtaining the samples of gastrointestinal juice with minimum incision should also be comprehensively investigated.

Demand response based congestion management in a mix of pool and bilateral electricity market model

摘要: The independent system operator (ISO) is a key element in the deregulated structure with one of the responsibilities of transmission congestion management (CM). The ISO opts market based solutions to manage congestion receiving bids from generation companies (GENCOs) as well as distribution companies (DISCOMs) to reschedule their generation and relocate demand. The nodal prices increases during the congestion hours and the demand response to nodal prices will be an effective tool for the control of congestion. In this paper, demand response-based CM has been proposed for a mix of pool and bilateral electricity market model. The linear bid curves have been considered for demand bids to respond to the congestion in the network. The bilateral demand has been obtained with minimum deviations in their preferred schedule. The impact of flexible alternating current transmission system (FACTS) devices viz static var compensator (SVC) and thyristor controlled series compensator (TCSC) has also been considered for demand management during congestion. Multi-line congestion cases have been considered to study the impact on demand response without and with FACTS devices. The proposed approach has been tested on the IEEE 24 bus test system.

Novel mutation c.1210-3C>G in with a poly-T tract of 5T affects mRNA splicing in a Chinese patient

摘要: Cystic fibrosis (CF) is a rare autosomal recessive disease with only one pathogenic gene cystic fibrosis transmembrane conductance regulator (CFTR). To identify the potential pathogenic mutations in a Chinese patient with CF, we conducted Sanger sequencing on the genomic DNA of the patient and his parents and detected all 27 coding exons of CFTR and their flanking intronic regions. The patient is a compound heterozygote of c.2909G>A, p.Gly970Asp in exon 18 and c.1210-3C>G in cis with a poly-T of 5T (T5) sequence, 3 bp upstream in intron 9. The splicing effect of c.1210-3C>G was verified via minigene assay in vitro, indicating that wild-type plasmid containing c.1210-3C together with T7 sequence produced a normal transcript and partial exon 10-skipping-transcript, whereas mutant plasmid containing c.1210-3G in cis with T5 sequence caused almost all mRNA to skip exon 10. Overall, c.1210-3C>G, the newly identified pathogenic mutation in our patient, in combination with T5 sequence in cis, affects the CFTR gene splicing and produces nearly no normal transcript in vitro. Moreover, this patient carries a p.Gly970Asp mutation, thus confirming the high-frequency of this mutation in Chinese patients with CF.

Expression status of GATA3 and mismatch repair proteins in upper tract urothelial carcinoma

摘要: GATA binding protein 3 (GATA3) and mismatch repair (MMR) deficiency contribute to the development of urothelial carcinoma. However, the combined expression of GATA3 and microsatellite instability (MSI) in upper tract urothelial carcinoma (UTUC) and its prognostic value have not been investigated. Here, we immunohistochemically stained GATA3 and MMR proteins in 108 UTUC samples. GATA3 was positive in 74 cases, and its expression was significantly lower than in adjacent benign urothelium ( <0.001). Loss of GATA3 expression was statistically associated with adverse clinicopathologic parameters, such as advanced stage, lymphovascular invasion, neural invasion, lymph node metastasis, and extensive necrosis. Cancer-specific survival (CSS, =0.028) and disease-free survival (DFS, =0.024) were significantly shorter in patients with GATA3 negative tumors than in patients with GATA3 positive tumors. The absence of MMR proteins was observed in 8.3% of the cases, and focal staining was identified in 13.0%. When using “lax criteria” which resulted in counting cases as negative where MMR staining was in fact focally positive (<5%), we found that GATA3 was inversely associated with MSI ( =0.005). Moreover, GATA3 /microsatellite stability (MS) tumors were correlated with advanced pT stage ( <0.001) and poor outcome ( =0.019 for CSS, =0.016 for DFS) compared with GATA3 /MSI ones. The GATA3 /MSI cases had unfavorable clinical outcomes compared with GATA3 /MSI cases ( =0.008 for CSS, =0.023 for DFS). This finding raises a question as to whether GATA3 interacts with MSI through the TGF- signaling pathway and regulates UTUC progression.

Oral product input to the GI tract: GIS an oral product performance technology

摘要: The patient receives a pharmaceutical product, not a drug. The pharmaceutical products are formulated with a drug, an active ingredient to produce the maximum therapeutic effect after oral absorption. Therefore, it is the product we must optimize for the patients. In order to assure the safety and efficacy of pharmaceutical products, we need an predictive tool for oral product performance in patients. Currently, we are a surprisingly long way from accomplishing that objective. If the 20th century was the ‘age of the drug’, i.e., the ‘magic bullet’, the 21st century must become the ‘age of the guided missile’, i.e., the delivery system, including the form of the active pharmaceutical ingredient (API) (‘drug’). The physical form of the drug and the delivery system must be optimized to maximize the therapeutic benefits of pharmaceutical products for humans. Oral immediate release (IR) dosage forms cannot be optimal for all drugs or likely even any drugs (APIs). Still, the formulation of pharmaceutical products has to be optimized for patients. But how do we optimize oral delivery of drugs? It is usually through ‘trial and error’, in humans! We need a better way to optimize the oral dosage forms. We have suggested to select different dissolution methodologies for this optimization based on BCS Subclasses. In this article, we present the predicted drug dissolution profile of ketoconazole as a model drug from our laboratory utilizing a gastrointestinal simulator (GIS), which is an adaptation of the ASD system. GIS consists of three chambers representing stomach, duodenum, and jejunum, to create the human gastrointestinal tract-like environment and enable the control the gastric emptying rate. This dissolution system allows the monitoring of the drug dissolution phenomena and the observation of the supersaturation and the precipitation of pharmaceutical products, which is useful information to predict dissolution of pharmaceutical products. This system can provide the actual input needed to accurately predict the input into the systemic circulation required by many of the absorption prediction packages available today.

摘要:

消化系统肿瘤包括食管癌、胃癌、结直肠癌、肝癌和胰腺癌,在全球的发病率和死亡率都很高。由于目前的肿瘤研究模型不能完全模拟原始肿瘤的关键特征,常导致基础研究不能转化用于临床实践。肿瘤发病机制的探索和抗癌药物的开发迫切需要更优化的肿瘤模型。类器官来源于组织和多能干细胞,是体外培养的三维自组织型的器官结构,准确地模拟了原始组织的组织学特征和遗传异质性。利用肿瘤患者的组织可以高效地建立正常和肿瘤类器官。本文总结并讨论了消化系统类器官的构建方法,及其作为一种新模型在基础癌症研究、临床前实践和个性化治疗中的应用。

Serologic study on the outbreak of acute upper respiratory tract infections caused by adenovirus 3

摘要: From April to June, 2004, an outbreak of acute upper respiratory tract infections (AURTI) occurred in the north area of Jiangsu Province, China. Twenty throat swabs were collected with 13 of them presenting an adenovirus (Ad)-like cytopathogenic effect on HEp-2. These were verified as Ad by the electron microscope, direct immunofluorescence assay and Ad primer-mediated PCR. Moreover, they were identified as adenovirus type 3 (Ad3) by type-specific PCR and sequencing of the amplification products. Subsequent serologic studies were carried out to finally diagnose and document the outbreak. The neutralization test of paired serum of six in nine cases show obviously increased antibodies titers. The positive rate of IgM, IgG and recovery phase neutralization antibodies of the cases were 3.7%, 44.4% and 59.5% respectively while those of the controls were 0%, 8.3% and 33.3% respectively. The values of Chi-Square were 0.510, 0.018 and 0.226 respectively. The concordance between IgG detected by ELISA and neutralization anti bodies detected by the neutralization test was 61.4% and the value of Kappa was 0.070. By the serologic study, we can definitively diagnose that this outbreak of acute respiratory infections was caused by Adenovirus 3.

乌鲁木齐市流动育龄妇女阴道炎患病现况调查

摘要:

了解乌鲁木齐市流动人口育龄妇女阴道炎感染患病现况,为今后的生殖道感染防治提供参考依据。本文采取分层-整群-抽样的方法,在乌鲁木齐市六区一县流动人口较集中的社区,由经过培训的医生对18~50 岁的流动育龄妇女进行生殖道感染问卷调查、妇科检查与实验室检查。本研究共检查106 568 名符合条件的流动妇女,其中细菌性阴道炎的患病率为16.15 %;滴虫性阴道炎的患病率为12.66 %;霉菌性阴道炎的患病率为4.52 %。3 种阴道炎的患病率为19.84 %。应根据不同民族的特点,采取一定的防治措施,降低流动育龄妇女的阴道炎患病率,特别是对文化程度为文盲或小学、职业为农民、月收入在1 500~1 599 元的对象,应加大干预的力度。

Attuned design of demand response program and M-FACTS for relieving congestion in a restructured market

摘要: This paper addresses the attuned use of multi-converter flexible alternative current transmission systems (M-FACTS) devices and demand response (DR) to perform congestion management (CM) in the deregulated environment. The strong control capability of the M-FACTS offers a great potential in solving many of the problems facing electric utilities. Besides, DR is a novel procedure that can be an effective tool for reduction of congestion. A market clearing procedure is conducted based on maximizing social welfare (SW) and congestion as network constraint is paid by using concurrently the DR and M-FACTS. A multi-objective problem (MOP) based on the sum of the payments received by the generators for changing their output, the total payment received by DR participants to reduce their load and M-FACTS cost is systematized. For the solution of this problem a nonlinear time-varying evolution (NTVE) based multi-objective particle swarm optimization (MOPSO) style is formed. Fuzzy decision-making (FDM) and technique for order preference by similarity to ideal solution (TOPSIS) approaches are employed for finding the best compromise solution from the set of Pareto-solutions obtained through multi-objective particle swarm optimization-nonlinear time-varying evolution (MOPSO-NTVE). In a real power system, Azarbaijan regional power system of Iran, comparative analysis of the results obtained from the application of the DR & unified power flow controller (UPFC) and the DR & M-FACTS are presented.

c-Fos expression in rat brainstem following intake of sucrose or saccharin

摘要: To examine whether the activation of brainstem neurons during intake of a sweet tastant is due to orosensory signals or post-ingestive factors, we compared the distribution of c-Fos-like immunoreactivity (c-FLI) in the nucleus of the solitary tract (NST) and parabrachial nucleus (PBN) of brainstem following ingestion of 0.25 M sucrose or 0.005 M saccharin solutions. Immunopositive neurons were localized mainly in the middle zone of the PBN and four rostral-caudal subregions of the NST. Intake of sucrose increased the number of FLI neurons in almost every subnucleus of the PBN (F = 7.610, = 0.023), in addition to the caudal NST at the level of the area postrema (F = 10.777, = 0.003) and the NST intermediate zone (F = 7.193, = 0.014). No significant increase in the number of c-Fos positive neurons was detected in response to saccharin ingestion, although there was a trend towards a modest increase in a few select NST and PBN nuclei. These results suggest that the PBN and NST may be involved in sweet taste perception and modulation of sweet tastant intake, but the significantly enhanced intensity of Fos expression induced by sucrose indicates that PBN/NST neuronal activity is driven by the integrated effects of sweet taste sensation and post-ingestive signals.

摘要:

提出了“城市基因改造论”和“畅通城市新模式”。目前城市“有机体”中存在着致病基因——“空间错位”,正是这一致病基因,导致产生了影响城市可持续发展的十大问题:城市占地多、效率低、能耗高、污染重、停车难、治安差、不宜居,以及交通事故多、交通投资巨大等。城市交通问题应该从“城市交通-城市空间-经济-社会”四个层次综合研究,“跳出交通看交通”,铲除“空间错位”这个基因,才能一揽子解决城市交通和城市可持续发展的诸多问题。通过分别构筑车与人的空间,将一个混乱的城市空间变为两个自由空间,从而构建“畅通城市新模式”,既可实现《雅典宪章》所提出的寻找新的街道系统以适应汽车交通特性[1],又可实现建设“以人为本”节约宜人型城市的新目标。

城市道路网运行实时动态评价理论和技术研究

摘要:

从道路网运行的基本特性入手,着重分析了路网运行的随机波动性、递延传导效应和周期规律性。在此基础上,剖析了迄今国内外一直沿用的道路“负荷度”路网评价理论的局限性,提出一套适用于路网整体实时动态评价的理论和技术方法,解决了无盲区实时数据采集与处理、路网运行时空动态分析、评价指标阈值标定等关键技术难题,为交通战略规划、实时动态路网功况诊断等提供了全新的技术手段,北京市的实证研究初步证明了所提出的理论和技术体系科学、有效、实用。
